Mesothelin is a cell-surface protein expressed by mesothelial cells. Mesothelial cells form a protective lining called the mesothelium, which surrounds the lungs, abdominal cavity, heart, and other internal organs. A soluble form can be measured in the blood and may serve as a marker of mesothelial injury.

Mesothelin measurement may have important diagnostic and prognostic value in cancer, particularly mesothelioma and ovarian cancer. Mesothelioma is a cancer that develops in the mesothelium, most often in the pleural mesothelium surrounding the lungs, and is usually linked to asbestos exposure. Elevated mesothelin levels may help distinguish mesothelioma from benign pleural conditions and may also be useful for monitoring disease progression. In ovarian cancer, mesothelin may support diagnosis and provide prognostic information, particularly when combined with CA 125. In non-malignant disease, mesothelin may be an indicator of mesothelial injury, particularly injury related to asbestos exposure.
